Psykiatrisk omvårdnad och psykiskt hälsoarbete i lokalsamhället - en resumé av internationell forskning och yrkesmässig reflektion
Abstract
Lokalsamhälle är ett komplext begrepp som omfattar både gemenskap
och utanförskap, hem och gräns. Ofta avses ett avgränsat geografiskt område med
formella och informella institutioner, värderingar, hierarkier, utvecklings- och
kommunikationsvägar. Lokalsamhället är den primära arenan för psykiskt hälsoarbete och
psykiatrisk omvårdnad. Syftet med denna litteraturresumé var att ge en bred översikt över
forskning och yrkesmässig reflektion relaterad till psykiatrisk omvårdnad och psykiskt
hälsoarbete i lokalsamhället. Forskningsfrågorna inriktades på centrala forskningsteman,
kunskapsutveckling och framtida utmaningar som beskrivs inom området från 1990 till
2002. Resumén bygger huvudsakligen på vetenskapliga artiklar vilka kategoriserats och
beskrivs kortfattat.
Tre centrala teman inom psykiatrisk omvårdnad och psykiskt hälsoarbete i
lokalsamhället har framkommit; Utvecklingen, historiken och professionaliseringen,
Förutsättningar, innebörd och praxis, samt Arbetets inriktning på personer eller grupper i
riskzonen för psykisk ohälsa eller på personer med långvariga och allvarlig psykisk
ohälsa och deras närstående. Kunskapsutvecklingen har huvudsakligen en deskriptiv
ansats med kvalitativa och kvantitativa datainsamlingsmetoder eller bygger på
yrkesmässig reflektion och diskussion. Framtidsfrågorna inriktas huvudsakligen på
behovet av att fördjupa kunskapen om specifik psykiatrisk omvårdnad och psykiskt
hälsoarbete som riktas till enskilda personer men även på behovet att påverka psykisk
hälsa hos utsatta grupper i befolkningen. Utveckling av psykiatrisk omvårdnad genom
handlingsorienterad forskning, handledning, utbildningsfrågor, samverkan och metoder
för utvärdering betonas liksom behovet av kunskapsteoretisk förankring av betonas. English: Community is a complex concept comprising fellowship and alienation,
home and border. Often a local community is conceived as a delimited geographic area
with formal and informal institutions, values, hierarchies and communications. The local
community is the primary arena of mental health work and psychiatric nursing. The aim
of this literature resumé was to give a broad overview of research and professional
reflection related to mental health work and psychiatric nursing in the local community.
The research questions focused on central research themes, knowledge development and
future challenges described within the field from 1990 through 2002. The resumé rests
mainly on scientific articles that are categorised and described briefly.
Three central themes have appeared: a) Development, history and professionalism, b)
Presuppositions, implications and practice and, c) Focusing persons and groups at risk of
developing mental unhealth or on persons with long-term and serious mental sufferings
and their relatives. The knowledge development has primarily descriptive designs with
qualitative and quantitative methods of data gathering, or rests on professional reflection
and discussion. To a large degree the questions related to future research focus on the
need to deepen the knowledge on specific mental health work and psychiatric nursing
towards single persons, but even on the need to promote mental health in vulnerable
groups in the population. The development of mental health work by means of
cooperative inquiry, supervision, training, collaboration and methods of evaluation are
emphasised - as well as the need for basic epistemological reflection.
