Sammendrag
Bakgrunn: Forskning viser at de fleste ønsker å dø hjemme. Til tross for dette dør kun 14% hjemme i Norge i dag. Etter samhandlingsreformen har kommunen fått mer ansvar, og flere alvorlig syke pasienter tilbringer mer tid hjemme. Kreftsykepleier i kommunen møter palliative pasienter i større grad enn før og det er viktig å fokusere på hva pasientene selv ønsker i denne fasen.
Hensikt: Belyse hvilke faktorer pasienter ser på som viktige for å kunne dø hjemme, og hvordan kreftsykepleier kan bidra som en del av organiseringen rundt pasienten.
Problemstilling: Hvilke faktorer må være tilstede for at kreftpasienten i sen palliativ fase velger å dø hjemme, og hvordan kan kreftsykepleier bidra for at dette kan gjennomføres?
Metode: Oppgaven er et litteraturstudie der vi bruker tidligere publisert litteratur, egne erfaringer og erfaringer fra praksisfeltet for øvrig.
Funn/diskusjon: Funn viser at pårørendes tilstedeværelse og omsorg er av avgjørende betydning for pasienter i palliativ fase. Videre er tillit, god informasjon, god symptomkontroll, kontinuitet og kompetanse blant personalet sett på som viktig for både pasienter og pårørende. I tillegg trekker pårørende frem forutsigbarhet, forberedthet og sykepleietilgjengelighet som vesentlig for deres trygghet. Som en følge av dette ser vi at organisering av tjenesten har stor betydning.
Konklusjon: For å imøtekomme pasient og pårørendes behov er det viktig at kreftsykepleieren kommer tidlig på banen. Gjennom sin koordinerende funksjon bør kreftsykepleier være delaktig i organiseringen rundt pasienten og fungere som en ressursperson i samhandling og kommunikasjon med fastlege, palliativt team og andre aktuelle aktører. I tillegg kan de bruke sin undervisende funksjon ved råd og veiledning til pasienter, pårørende og andre kollegaer. På denne måten formidler kreftsykepleier trygghet og kompetanse. Engelsk sammendrag (abstract): Background: Research indicates that most people want to die at home. Despite this only 14% dies at home in Norway today. The municipal responsibility has increased as a result of “samhandlingsreformen” (coordination reform), and the seriously ill cancer patients are now spending more time at home. The oncology nurses in the districts meet an increasing number of palliative patients and need to focus on the patient's wishes in this phase of life.
Aim: Identify the factors patients see as important to be able to die at home and find out how the oncology nurse can help as part of the organization around the patient and the relatives.
Problem: What are the factors that must be present for cancer patients in the late palliative phase to choose to die at home, and what can the oncology nurse do to facilitate the implementation of the patient's choice?
Method: The thesis is a literature study where we in addition to previously published literature; take in to consideration both our own experiences and lessons learned from the practice field at large.
Findings/Discussion: The findings show the significance of the presence and care given to the patient by his or her next of kin in the palliative phase. Furthermore, trust, sufficient information, satisfactory control of symptoms, continuity, as well as competence among the staff, are seen as important for both the patient and the people standing closest to him or her. In addition, as emphasized by the next of kin, predictability, preparations and accessibility of nurses are crucial for their sense of security. As a consequence, we recognize the great importance of the organization and coordination of the service.
Conclusion:
To meet the needs of both patient and relatives it is important that the cancer nurse is present at an early stage. Through her function as coordinator the oncology nurse should take part in the organization around the patient and be a person of resource to the cooperation and communication with the physician, the palliative team and other potential participants. She may also use her function as an educator to give advice to patients, relatives and colleges. This way the oncology nurse will impart security and competence.
