dc.description.abstractSamhandlingsreformen skal bidra til å nå den norske velferdsstatens viktigste og vanskeligste mål; å sikre gode og lett tilgjengelige helse- og omsorgstjenester til alle som har behov for det, og helst i eget lokalmiljø. Hensikten med avhandlingen er å kunne komme fram til faktorer som representanter fra kommunene selv mener er viktige for at små kommuner skal lykkes med å implementere og transformere samhandlingsreformen, og hva som er til hinder for å lykkes. Jeg har gjennomført en kvalitativ studie av 10 kommuner i Finnmark fylke, der jeg har spurt representanter fra kommunene om deres erfaringer og synspunkter om samhandlingsreformen. Dybdeintervju og observasjon er benyttet som metode. Undersøkelsen kan gi en bedre kunnskap og en hjelp til å forstå hvordan interaksjonen foregår mellom aktørene når de er nødt til å samhandle for å innfri krav i reformen. Målgruppen for arbeidet er politiske og administrative ledere, beslutningstakere i kommunene, og de operative aktørene som får befatningen med reformen. Framgangsmåten har vært å få tak i meningsinnholdet i representantenes oppfatninger av kommunenes muligheter for å kunne oppnå samhandlingsreformens intensjoner. Undersøkelsen avdekker at det er meningsforskjeller og motsetningsforhold innenfor kategoriene tillit, ledelse, konkurranse og ressurser. I tillegg det er motsetninger mellom politisk, administrativt og operativt nivå. Representantene spriker i virkelighetsforståelsen noe som vil hemme samhandling både internt og eksternt mot andre kommuner. Analysen viser at tillit og ledelse er de faktorene som representantene mener er de mest viktige for å lykkes. Maktbalansen bør være gjennomtenkt og det er viktig at representantene bruker tid til å bli godt kjent med hverandre og avstemmer forventninger. Et interessant funn ved undersøkelsen er at intensjonen til representantene er gode, men disse samsvarer ikke alltid med hva som skjer når en møter praksisen og hverdagen i kommunene. Bygdedyret som metafor er aktivt i bruk, og det ser ut til å spille på flere funksjoner i lokalsamfunnet. Jeg mener undersøkelsen gir gode innspill til små kommuner i Finnmark og kan være en ”ide-bank” for hva som fremmer og hemmer samhandling for å kunne nå intensjonene i samhandlingsreformen.no_NO
dc.description.abstractEngelsk sammendrag (abstract): The Health Care Interaction reform will help Norway to achieve its most important and challenging goal as a welfare state: to ensure good and accessible health care to everyone who needs it, preferably in their own local community. The main goal of this thesis is to arrive at a conclusion that representatives from the municipalities themselves play the key part in achieving successful implementation and transformation of the interaction reform, I have conducted a qualitative study in 10 municipalities in the Finnmark county where I have asked the representatives of each municipality to further elaborate on their experiences and views regarding the interaction reform. I used in-depth interviews and observation as the main method. The study provides a better understanding on how interaction takes place between the players whose goal is to fulfil the requirements of the reform. The target group for this thesis is the political and administrative leaders, decision-makers in the municipalities, as well as the operational actors involved in the reform. The survey among the representatives of 10 municipalities in Finnmark reveals that there are differences of opinion and conflict when it comes to trust, leadership, competition and resources. In addition, there is miscommunication between the political, administrative and operational actors. These differences of opinion have become the main hurdles to inhibit smooth collaboration both internally and externally. The analysis shows that the officials believe that trust and leadership are the main factors of success. The balance of power should be considered and it is important that representatives take the time to get to know each other and manage expectations. Interestingly, the survey shows that the intentions of the directors are good, however they do not always correspond to everyday practice. Overall, the study provides valuable contributions to small municipalities in Finnmark and can be used as an "idea bank" for what it takes to reach the spirit of collaboration for a successful implementation of the reform.en
