Den planlagte miljøterapeutiske samtalen – delrapport i Miljøterapiprosjektet

Abstract

Norsk: Miljøterapiprosjektet er et forskningssamarbeid mellom Sykehus Innlandet, Divisjon Psykisk helsevern og Høgskolen i Hedmark. Denne studien er utviklet i samarbeid med en av enhetene som er med i miljøterapiprosjektet. Enheten vil utvikle sin kompetanse innen psykosebehandling og psykososial rehabilitering til pasienter med alvorlig psykisk lidelse og et samtidig rusproblem. Fokus i denne behandlingen og omsorgen vil være å skape et godt miljø for personer som har behov for lengre tids behandling og rehabilitering. Dette forutsetter at det miljøterapeutiske tilbudet utvikles blant annet gjennom å vektlegge den miljøterapeutiske samtalen. I en miljøterapeutisk tilnærming er det viktig å sikre at samtaler med pasientene skal være meningsfylte. Hensikten med denne studien er å få en dypere forståelse for og videreutvikle den miljøterapeutiske samtalen. Handlingsorientert forskningssamarbeid skal fremme den faglige kompetansen hos ansatte gjennom systematisk refleksjon over praksiserfaringer og medskapning av relevant kunnskap. Flerstegs fokusgruppeintervju ble brukt som datainnsamlingsmetode. Tilsammen 12 medforskere/respondenter deltok i de fire fokusgruppeintervjuene høsten 2008. Kvalitativ innholdsanalyse ble benyttet som analysemetode. Innholdet i den planlagte miljøterapeutiske samtalen i enheten har blitt identifisert: Det selvstendige livet, lidelsen, livserfaringer og emosjonelle tilstander. Samtalens plattform knyttet seg til holdninger som bærer samtalen, holdninger til brukermedvirkning og holdninger til rusproblemer. Brukermedvirkning er et område som medforskerne i miljøterapiprosjektet har satt et økt fokus på, og som de i det framtidige arbeidet ønsker å videreutvikle. Som et godt hjelpemiddel i samarbeidet med pasientene og spesielt i planlagte miljøterapeutiske samtaler viser studien at sykepleieplanen er et godt verktøy. Denne ble revitalisert på enheten. Funnene i studien viser at personalet på enheten gjennom granskningen har fått en dypere forståelse for betydningen av planlagte miljøterapeutiske samtaler. Det vil være viktig å tilstrebe og utvikle dialogen mellom pasient og hjelper i en retning som har likeverd, akseptasjon, varme og empati som verdier. Når samhandlingen mellom miljøterapeut og pasient skal være terapeutisk i et helsefremmende perspektiv, bør samarbeidet i størst mulig grad være preget av et arbeid som legger vekt på likeverdighet, ansvarliggjøring og selvstendiggjøring.

English: The Milieu therapy project is a research cooperation between Innlandet Hospital Trust, Division of Mental Health Care and Hedemark University College. This study was developed in collaboration with one of the units which is part of the milieu therapy project. The unit wants to develop its expertise in psychosis treatment and psychosocial habilitation for patients with serious mental illness. The focus in this treatment and care will be to create a good environment for people who need prolonged treatment and rehabilitation. This requires that the milieu therapeutic environment, among other things, offers to emphasize the provision of milieu therapeutic conversation. In a milieu therapeutic approach, it is important to ensure that dialogues with patients should be meaningful. The purpose of this study is to gain a deeper understanding about, and to further develop, the milieu therapeutic dialogue. Co-operative inquiry-oriented research shall promote the professional competence of employees through systematic reflection on practice experience and the fellow creation of the relevant knowledge. The multi-stage focus-group interview was used as a data collection method. A total of 12 co-researchers/ respondents participated in the four focus group interviews in autumn 2008. Qualitative content analysis was used as analysis method. The content of the planned milieu therapeutic dialogue in the unit has been identified as: the independent life, the suffering, life experiences and the emotional states. The dialogue platform linked itself to the attitudes that carry the conversation, attitudes to the user-participation, and attitudes to the drug problems. User-participation is an area that co-researchers in The Milieu therapy project has had a greater focus on, and that they in the future work wants to develop further. As a helpful tool in collaboration with patients and especially in the planned milieu therapeutic dialogues, the study shows that a special nursing-planning is a good tool. This was revamped for the unit. The findings of the study show that the personel at the unit through the investigation has been given a deeper understanding of the importance of the planned milieu therapeutic dialogues. It will be important to maintain and develop the dialogue between patient and the helper in a direction that has equality, acceptance, tenderness and empathy as values. When the interaction between the milieu therapist and the patient should be therapeutic in a health promotion perspective, the cooperation should, in the greatest extent possible, be characterized by a work that emphasizes equality, responsibility and to be independent.
