Det indre og ytre blikk for skam ved overvekt og fedme - en kvalitativ studie av helsepersonell.

Abstract

Sammendrag Bakgrunn: Helsepersonell i behandlingsrelasjon med pasienter som strever med overvekt og fedme, skal bidra til mestring av sykdommen og utfordringer de står ovenfor. Fedme og overvekt er ofte forbundet med skam, men skam er derimot ikke tematisert og vektlagt i nasjonale retningslinjer. Vi mener skam er vesentlig å forholde seg til i en behandlingsrelasjon, og ønsket derfor å utføre en studie for å undersøke dette nærmere.

Hensikt: Hensikten har vært å få en større forståelse og økt kunnskap om hvordan helsepersonell forstår og møter skam i behandlingsrelasjonen, hos pasienter med overvekt og fedme.

Metode: Vi har benyttet kvalitativ metode. Studien har et eksplorerende og beskrivende design. Vi har gjennomført åtte individuelle semistrukturerte intervjuer av helsepersonell med ulik profesjon. Systematisk tekstkondensering (STK) er brukt som analysemetode.

Resultater: Informantene hadde god forståelse av skam. Informantene hadde ulike strategier i hvordan de møtte pasienter med skam på, derimot viste studien at informantene sjelden tok ordet «skam» i tale selv om de avdekket at pasienten led av skam. Informantene så nytteverdien av å tematisere skam som del av behandlingen, også med synlighet på nasjonalt nivå.

Konklusjon: Informantene hadde gode verktøy for å møte skam i behandlingsrelasjonen, hos pasienter med overvekt og fedme. Videre viste de god forståelse av skam, selv om skam ikke sto på dagsorden i behandlingen. Med tematisering av skam i de nasjonale retningslinjene for behandling av overvekt og fedme i Norge, vil det kunne bidra til å synliggjøre viktigheten av å løfte opp temaet skam i behandlingen, og gi helsepersonell andre rammer å jobbe etter.

Abstract Background: Health-personnel in a treatment relationship with patients struggling with overweight and obesity must contribute to mastering the disease and the challenges they face. Obesity and overweight are often associated with shame, but shame, on the other hand, is not thematized and emphasized in national guidelines. We believe that shame is important to deal with in a treatment relationship, and therefore wanted to carry out a study to investigate this.

Aim: The purpose has been to gain a greater understanding and increased knowledge of how health-personnel understand and encounter shame in the treatment relationship, in patients with overweight and obesity.

Method: We have used a qualitative method. The study has an exploratory and descriptive design. We have conducted eight individual, semi-structured interviews of health-personnel with different professions. Systematic text-condensation (STK) is used as an analysis method.

Results: The informants had a good understanding of shame. They had different strategies in how they met patients with shame, on the other hand, the study showed that the informants rarely mentioned the word “shame” even if they discovered that the patient suffered from shame. The informants saw the utility of thematizing shame as part of the treatment, also with visibility at national level.

Conclusion: The informants had good tools for addressing shame in the treatment relationship, in patients with overweight and obesity. Furthermore, they showed a good understanding of shame, even though shame was not on the agenda in the treatment. By addressing shame in the national guidelines for the treatment of overweight and obesity in Norway, it will help to highlight the importance of raising the topic of shame in treatment, and give health personnel other frameworks to work by
